CliniCom™ Reports
CliniCom™ generates instant on-demand reports that include all of the components of a complete medical history. This includes patient demographics, chief complaint, history of present illness with associated symptoms, developmental history, current and past treatment and medication history, family history, and social history, and finally suggested diagnosis that should be considered for further evaluation. Suggested diagnosis include severity values and their corresponding ICD-9CM codes. CliniCom™ also allows for better communication with referral sources by allowing you to generate easy to understand reports and narratives that can quickly be sent to those sources.
Sample Reports
These documents are samples of actual CliniCom™ reports. These reports have been de-identified in compliance with HIPAA privacy policies. These reports are similar to what all CliniCom™ Doctors see when their patients complete the CliniCom™ digital intake and assessment. These sample reports help demonstrate the capacity that CliniCom™ has for thoroughly assessing patients for mental illnesses. Keep in mind that while the report format remains the same, the depth and scope of each CliniCom™ report varies based on the patient's condition, the severity of their case, and the reliability of the informant.
Sample 1: Child - Depression - Female
This is a 15 year old female presenting with depression, hopelessness and hallucinations. She has tried a few medications in the past and has been hospitalized twice for psychiatric care. She has experienced sexual abuse in the past. Has side effects from current medication.
This is a 42 year old prior military male presenting with sleep problems. He has experienced one year of combat.
